Forms and information required for the 2010-2011 soccer season.
1) Registration/Commitment form - Please complete this form prior to or at tryouts for the 2010-2011 season. This form must be provided to the club prior to or at tryouts (prior to tryouts if you’re unable to attend). After tryouts this form is held by the club for record keeping purposes.
2) Medical Release Form - Please complete this form prior to or at tryouts for the 2010-2011 season. This form must be provided to the club prior to or at tryouts (prior to tryouts if you’re unable to attend). We will have a notary at our tryouts to complete and finalize the medical release form. After tryouts this form is held by the team manager to allow for proper care in the event of an injury.
3) Birth Certificate- If this is your first season with the Fever Academy please bring a copy of your child’s birth certificate. After tryouts this form will be kept by the team manager for record keeping purposes at all games and tournaments.
4) Wallet Size Photo - Please bring a wallet size photo to tryouts. This will be used to create your player card.
5) Registration Fee - Please be prepared to pay the $195 registration fee at tryouts. This fee must be paid in order for the player to accept a position for his/her respective team. You may pay the registration fee by cash, check, visa, mastercard, or discover.
6) Completed ACH form or Credit Card Authorization form.
