We want to join the Fever - What do we do next?

We hope you will join the Fever Family. In order to join for the 2011-12 season, please follow the step and forms provided below and to the left.

1. Please bring the following forms and information to Registration Day at the Plex North.

• Completed Registration and Commitment Forms
• Completed Medical Release Form (Notary will be provided at Registration)
• Completed Payment Authorization Form
• Completed Financial Assistance Form (if applicable)
• A $150 non-refundable signing fee along with the 1st month’s payment
• A wallet size photo
• A copy of player’s birth certificate
