We hope you will join the Fever Family. In order to join for the 2012-13 season, please bring the following items to Trish (Plex North) or Nancy (Plex South):

• Commitment Form (Completed & Signed) Mandatory
• Payment Authorization form (completed -either credit card of ACH) unless paying in full Mandatory
• Refund/Payment Policy Page (signed) Mandatory
• Plex Roster Waiver Form Mandatory
• A wallet size photo
• A copy of player’s birth certificate
• Deposit payment (first and last month's installments)
